All patients admitted with a stroke to the participating hospitals should receive a copy of an Australian Stroke Clinical Registry Patient Information Sheet. If you have not received this document, you can download the copy relevant to your hospital using the link below.
Each Patient Information Sheet is tailored specifically to a hospital’s ethical requirements and provides information on the purpose, management, data collection items, how information is kept confidential, and risks and benefits to patients.
